Maintaining Stability and Safety

Crisis Care Navigation is a short-term, community-based program that helps youth and families maintain safety and stability during transitions in care. We support youth who are entering or leaving hospitals, residential programs, crisis units, or other higher levels of care, as well as families who are unsure what type of support they need next.

Our team meets families where they are, providing individualized support in their homes and communities while helping determine the most appropriate path forward.

Stabilize

We help youth and families address immediate challenges and restore stability in their everyday environment. Support may address trauma, mental health concerns, escalating behaviors, suicidal thoughts, caregiver conflict, and other factors affecting safety and well-being.

Assess

When the right level of care is unclear, our team works with youth and caregivers to understand their needs, assess safety, and identify the supports most likely to help them succeed.

Connect

We help families navigate the care continuum and create a warm connection to appropriate ongoing services. Whether a youth is transitioning home from a hospital or residential program or needs additional support to prevent a higher level of care, we help families understand their options and connect with the next step.

By providing support in the youth’s natural environment, Bellefaire's Crisis Care Navigation can help families maintain stability while reducing unnecessary disruption and supporting successful transitions between levels of care.

The goal is simple: help youth and families move through moments of crisis safely, confidently, and with a clear path to continued support.
